Pay for your Appointment via Patient Portal
Use the following link to open the CCCC Patient Portal website. Your balance, if owed, will be displayed on your dashboard. Your username is set up as your email address, if enabled. If you do not yet have Patient Portal access, please contact us.

Pay for your Appointment via Consent
On the second page of the Telehealth Informed Consent, there is a section for you to fill in your debit card or credit card information, just above your signature. If you have completed this section and given us your card information, our billing team will automatically run your payment after confirming with your clinician that your Appointment occurred as scheduled.
Pay for your Appointment via Phone
If you have not given us your card information via Consent, you may call us to to leave a card on file for next-day payment processing after each appointment and to pay any outstanding balance.
